Health and medical details: GP surgery and contact details, emergency contact details, health conditions and medication.

Session Notes: Brief notes taken during our sessions outlining the themes, goals, and content of our work

Purpose of collecting data

For administration purposes, for example in cases of cancelling or rescheduling sessions.

For safeguarding purposes, in case of high risk safeguarding concerns being raised.

Confidentiality

Information shared in therapy is treated confidentially, except where there is a legal, ethical, or safeguarding reason to share information.

How Long Information Is Kept

Records are retained in accordance with professional and legal requirements and are securely destroyed when no longer required.
